What we will require from you.
Everyone entering the office will be required to wear masks that correctly cover the mouth and nose at all time.
(Gloves are optional as protective personal equipment.)
Adult patients are to attend their appointment by themselves.
Children and those with guardians are allowed only one parent or guardian to attend.
You will be completing a brief health screener before entering the office which will include answering questions related to travel, your health status, and the possibility of exposure to those who are ill.
You will then have your temperature taken with a head scanner.
Finally, you will be asked to wash your hands.
PLEASE BE AWARE THAT THE OFFICE DOOR WILL BE LOCKED UNTIL YOUR SCHEDULED APPOINTMENT TIME. WE WILL OPEN AND PERFORM THE SCREENING BEFORE YOUR APPOINTMENT. THIS ENSURES SOCIAL DISTANCING AND LIMITS EXPOSURE AS MUCH AS POSSIBLE.

WHAT TO EXPECT AT YOUR VISIT
Please be sure to thoroughly read the HIPAA tab for answers to commonly asked questions. Completion and signing your initial forms indicates that you have done this and understand the presented information. If you ask questions that have already been addressed in this section and the HIPAA section, we will refer you back to the website to re-read the information.
Please note that we reserve the right to reschedule your appointment if you arrive late in consideration of other patients. If no communication is provided to us, a late appointment will be considered to be equivalent to a missed appointment without notification.
Please also note that all scheduling is done in writing via email with text alerts. This is part of our office policies to assist our patients as well as to allow for documentation.
For all appointments: If you or your child or you are sick, please inform us as soon as possible. We can reschedule your appointment and maintain a healthy environment, as some of our patients are immune compromised.
CNS, PC has a zero tolerance policy for profanity, verbal threats, harassment, or acts of violence of any kind. This behavior will not be tolerated, and this will result in immediate termination of services with CNS, PC regardless of the stage of completion or progress in the evaluation or treatment. Staff and patients have the right to a safe, serene, and respectful environment at all times. Most people respect and follow these guidelines for comportment. By engaging in services with CNS, PC, you are agreeing to be one of them.
- For treatment
The first session will be an initial evaluation where background information is obtained and goals of treatment are developed. This usually takes 1 hour. Follow up sessions for treatment are scheduled for 50 minutes.
- For Assessment
1. The first session will be an initial evaluation for approximately one hour where the paperwork you have submitted will be reviewed in detail. This session will also involve a neurobehavioral evaluation and mental status exam. This is essential to complete so that we can acquire prior authorization from your insurance company. After this initial evaluation, you have not completed the evaluation process. Please continue to read carefully the process so that you know what to expect.
2. After the initial visit, we will then submit for authorization for testing to your insurance company. Depending on the company, this may take some time (weeks to months) to hear a response. Unfortunately, this is out of our control and we thank you in advance for your understanding and patience. Upon receiving authorization for testing, we will contact you by email and text to schedule the first testing appointment.
The following evaluation session is usually scheduled depending on what the individual can tolerate and availability. The specifics and timeframe of the appointment will be provided to you during scheduling. Breaks will be provided as deemed necessary. Individuals are welcome to bring refreshments and snacks to the appointment.
We intend to obtain the best and most comprehensive results from testing, so individuals will be evaluated in a stimulus free room with the examiner only present. We will schedule follow up sessions as necessary to complete the required testing. The time specifics will be provided during scheduling.
Please note that we may ask you to briefly discontinue certain medications that can confound your results during a testing session and we ask that you coordinate this with your prescriber in order to obtain a true baseline of functioning. This is optional, but will provide clearer results if completed and possible.
3. After the entire evaluation sessions are completed, our office will contact you to schedule a one hour feedback session where we discuss the results in meaningful terminology. There will be an opportunity to ask questions at this time and discuss treatment recommendations. This will be considered the completion of the neuropsychological assessment.
